New Genetic Test Could Help Transplant Patients Get the Right Anti-Rejection Drug Dose Faster

Finding the right medication dose after an organ transplant is critical, and often complicated. 

A new test launched by Thermo Fisher Scientific aims to make that process easier by using genetics to guide how doctors prescribe tacrolimus, one of the most commonly used anti-rejection drugs. 

Tacrolimus helps prevent the immune system from attacking a newly transplanted organ. But dosing it correctly can be tricky. 

  • Too little, and the body may reject the transplant. 
  • Too much, and patients face higher risks of infection, toxicity, and other serious complications. 

One of the reasons dosing is so difficult is that people process the drug differently, partly because of their genetics. 

The new TacroType™ Pharmacogenetic Test looks at a gene called CYP3A5, which affects how quickly a patient’s body breaks down tacrolimus. By identifying a patient’s genetic profile, the test can help doctors estimate how much medication a patient may need from the start. 

The test itself is simple: it uses a cheek swab sample and is designed to provide clinicians with genetic insights before or shortly after a transplant. 

Instead of relying entirely on trial-and-error adjustments in the first weeks after surgery, doctors could use this genetic information to make more informed dosing decisions early on, potentially helping patients reach safe drug levels faster. 

Experts say tools like this reflect a broader shift toward personalized medicine, where treatments are tailored to each patient’s unique biology. 

While the test doesn’t replace standard monitoring of drug levels, it adds another piece of information that may help clinicians manage immunosuppressive therapy more precisely during one of the most critical periods after transplantation.
